How do I schedule an appointment.

What you need to know.
When contacting the office for the first time, we will collect any information necessary for us to perform a benefits check with your insurance company. Within one business day, we will contact you to discuss any details of your benefits, including referrals, copayment, and deductibles. At this time, we will be able to schedule your first appointment, and answer any questions you may have. For any uninsured patient, or any patient who chooses not to use their insurance, we offer cash plans as an alternative.
What to expect.
All sessions are by appointment only. For your first appointment, be prepared to spend up to an hour and a half with the doctor. Every appointment thereafter will be about one hour, depending on your individual treatment. Please keep in mind that you are required to fill out a new Patient Intake Form, and to arrive about 10 minutes earlier than your appointment time to comeplete this. What to bring.
If you completed the intake form in advance, please bring it with you for your first appointment, as well your insurance card and driver’s license. If a referral is required by your insurance, please obtain that before your first visit and bring it with you (You can also have your doctor fax it to our office), along with any MRI or X-Ray reports pertaining to your injury. If you are covered by Workers’ Compensation or Auto Insurance, make sure you have your claim information and case manager or adjuster’s contact information.
How to dress.
Dress in loose, non-restricting clothing. You may bring a change of clothes if you are unsure of what will be the best for your treatment. We have cubbies to hold any extra belongings you may have brought.
What will my treatment entail?
Your first appointment will consist of an evaluation by the doctor to determine your diagnosis and the best treatment plan for optimal results. Based on the evaluation, the doctor will use different Physical Therapy techniques, which may include manual therapy, electrical stimulation, and therapeutic exercises. Depending on your diagnosis, pain levels, and other factors discussed during your initial evaluation, you may need just one treatment, or you may need weeks of treatment to reach your full healing potential.
